SO2 (£36,124 - £40,476) - starting salary of £38,626 for applicants who have been qualified for two years or more, and have passed their Practice Observation and Progression (POP) assessment’
37 hours, Full-Time
Temporary (12 months)
Hybrid: Baker Street and homeworking. Expectation that in the office for duty and Team Meetings.

Your Role
We are seeking a dedicated and compassionate Social Worker to join our team for a 12-month contract. In this role, you will be responsible for conducting comprehensive reviews for individuals eligible for Section 117 aftercare, following their support under the Mental Health Act. Your expertise will ensure that our clients receive the necessary care and support to thrive in their communities..

- Must be registered with Social Work England
- Must be eligible to work in the UK
- Experience of working with vulnerable adults within a hospital setting preferred
- Experience of report writing; such as social care assessments
- Must have experience of completing Care Act Assessments and support plans
- Experience of investigating and chairing safeguarding meetings preferred
- Experience of managing complex cases
- Experience of liaising with other professionals and agencies
- Experience of arranging safe and timely discharges for service users
- Familiar with self-directed support / personal budgets
- Familiar with CHC and Section 117 eligibility and applying for funding
- Experience of using Liquid Logic would also be beneficial but not essential
