Your Role

We are looking for a passionate and enthusiastic Assistant Team Leader, who holds a Social Work or Occupational Therapy degree and is registered with Social Work England/ HCPC to join The Front Door team.

The Front Door team consists of Occupational Therapists, Social Care Officers, Referral and Information Officers and Social Workers. Together the team are the first point of contact for residents and professionals who need advice and support from a range of services, agencies or community offers. The team works closely with partners such as NWAS, GMP, Stockport Family and Adult Social Care teams to triage and prioritise referrals to ensure people receive a timely and appropriate response to their enquiry. The OTs and SCO’s carry out short-term interventions to reduce the need for formal care. The Social Workers are responsible for statutory and non-statutory safeguarding enquiries, attendance at Team Around the Adults meetings and additional screening of referrals and hold a small caseload.

As an Assistant Team Leader you will need to undertake all statutory duties in line with the Care Act 2014 and other relevant legislation ensuring that the principle of promoting and supporting wellbeing is at the heart of your practice. You will work alongside the existing Assistant Team Leader and Team Manager to manage the day-to-day work of the team. This includes:

  • Triage and prioritise referrals. Thinking Prevent, Reduce, Delay.
  • Being confident in decision making in a fast-paced environment.
  • Allocation of case work.
  • Complete supervisions of staff and support through put of work, wellbeing and training/ development opportunities.

This supportive team is committed to multi- disciplinary working where each team member is valued for their unique contribution.

You will be a team player who has a keen interest in achieving better outcomes for adults we work with. You will bring your excellent organisational skills to the role. In addition, you will have the ability to be creative and good at problem solving.

To be successful in this role, you will have a can-do outlook, be flexible, enjoy fast paced work and multidisciplinary working. Have a good working knowledge of Stockport values, and expectations.
